Sports Medicine Athletic Clearance Checklist for Returning Students

1. Fill out a new Participation Statement on ATS by August 1st
   1. View ATS website
   2. Please email your athletic trainer if you do not know your login info, and once they have looked it up for you, please change it to your Potsdam email login info
2. Fill out a new Concussion Statement on ATS by August 1st
3. Verify Emergency Contact and Insurance Info on ATS is accurate by August 1st (there is a Verify button on each page on ATS)
4. Submit the Potsdam Athletics Physical form and Questionnaire by mail (make a copy first), fax, attach as a Form on ATS, or in-person by August 1st (Do Not Email)
