Campus reporting forms provide members of the SUNY Potsdam community the opportunity to share information with appropriate campus offices. Please note that these forms are not monitored 24/7 and are intended to address concerns in a safe, timely and supportive manner.
If there is an immediate or ongoing threat to the safety of an individual or the campus community, please contact University Police at (315) 267-2222.
After you submit the report, it will be routed to the appropriate campus office for review.
- Academic Integrity Reports are routed to the Office of Student Conduct and Community Standards (Academic Honor Code)
- Bias Reports are routed to the campus Bias Incident Response Team or BIRT (Bias Incident Reporting)
- Case Management Referrals and Case Management Walk In/Consult are routed to the campus Case Manager (Case Management)
- Sex Based Discrimination/Sexual Misconduct Reports are routed to the Campus Title IX Coordinator (Title IX)
- Student Conduct Reports are routed to the Office of Student Conduct and Community Standards (Student Conduct & Community Standards)
- Student of Concern Reports are routed to the campus Behavioral Evaluation Committee (Behavioral Evaluation Committee (BEC))
