Accident/Injury Report must be completed for all injuries. The requested information is required by law, under the NYS PESH Rule Part 801. When an employee is involved in a work-related injury, the following steps must be followed:
Documentation Guidelines
Accommodative Services will consider all documentation submitted. Documentation will assist in understanding how the disability impacts the student in an academic setting and the current impact of the disability as it relates to the accommodations requested. Reviewing documentation is a collaborative process and is handled on a case-to-case basis. Once documentation is received, it will be reviewed in order of receipt; therefore, it is recommended that documentation be submitted well in advance of any accommodation related needs.

Research Involving Human Or Animal Subjects
According to Federal and State law the Research Foundation, and College policy any research proposal involving the use of human or animal subjects must receive the approval of the campus Human Subjects Institutional Review Board or the Institutional Animal Care and Use Committee as appropriate prior to submission to a sponsor or implementation of the research project. The purpose of this policy is to protect the rights and welfare of human subjects and to insure humane treatment of animal subjects.

Airfare Procedure
In accordance with the Office of Government Relations, airfare purchased using State, IFR or Dorm funds must be arranged through the New York State Contract vendor and purchased using the traveler's JP Morgan Chase Travel Card. Airfare not purchased through our State contract vendor will NOT be reimbursed to the traveler.
