Forms & Documentation
ALL Office of the Registrar forms and supporting documentation should be uploaded using our secure file drop! Please do not email any sensitive personal information such as your social security number, driver's license, birth certificate, etc. Should you be asked to provide such information, upload sensitive documentation using the Office of the Registrar’s file drop where it will remain secure, encrypted and confidential.
Note: Uploading sensitive documents does not automatically alert Registrar staff. Please email registrar@potsdam.edu after uploading a document to alert Registrar staff.
Before submitting please be sure to:
- Include Student’s Name and Potsdam Identification Number (P#) in the file name.
- * Documents that do not reference the student’s name and P# in the file name will be processed after others.

- Retain a copy of the document for your records

- Academic Forgiveness Petition
The intent of the Academic Forgiveness policy is to allow readmitted students who previously accrued a record with a substantial number of grades below 2.0 to be “forgiven” for their earlier performance, if they meet the stated criteria. - Add/Drop Form

Complete this form to appeal a course grade. - Application for Bachelor's or Master's Degree
Students must file an Application for Degree during the semester prior to their final semester or ASAP in order to graduate. You can find the application in BearPAWS>Student Services & Financial Aid>Student Records>Apply to Graduate. - Citizenship Change Form
