Please note that prior to a semester schedule going live, changes are made via the CPI during the 1st and 2nd iterations. Once a schedule is live for a term, you should use the appropriate form below.
Please use the appropriate launch button below to Add, Revise, or Cancel a course in the Catalog, the Class Schedule, or both. Your request will be routed to the appropriate Dean’s office for review. If approved, it will then go the Registrar’s office for processing. If your request pertains to a Travel or Distance Learning course, it will also be routed to the appropriate coordinator for review (Note: a course should not be both Travel and Distance Learning.)

Faculty Governance Hour Exemption Form
This form can be used to apply for the Faculty Governance Hour Exemption. The Officer-in-Charge of the college has approved the SUNY Potsdam Faculty Senate's resolution of October 27, 2022, to prohibit scheduling of Fall and Spring semester classes during the faculty governance hours, Thursdays from 4:00 p.m. to 5:00 p.m. and Fridays from 3:00 p.m. to 4:00 p.m. Exceptions to this policy are possible only with the approval of the appropriate dean. This policy will go into effect for the Spring 2023 semester schedule.
FERPA Employee Confidentiality Form
This form can be used by offices of the college to ensure that employees (including work-study students) understand the regulations concerning the confidentiality of student educational records covered under the Family Education Rights and Privacy Act.
Grade Change
This form is used to submit a grade change for a data entry error, computation error, or after a grade appeal. Do NOT use this form to submit late grades after the posted semester grading deadline.
Instructor Drop Form
Students must attend the first meeting of each of their registered classes or contact the instructor within 48 hours of the beginning of the first meeting. If a student misses the first meeting without an explanation, the instructor may choose to drop the student from the section. Use this form to Instructor Drop a student from a class.
Opt-Out of Online Add/Drop Form
Use this form to request that a course be set up in a Part Of Term (POT) which will not allow for student self-service Add/Drop in BearPAWS.

This form can be used by departments for overrides during the registration period. This option opens another window.
Request for Access to BearPaws Administrator and Staff Reports
Our goal is to protect the security and confidentiality of student records, as required by the Family Educational Rights and Privacy Act (FERPA). This form will help us to protect student records, while allowing access to the information faculty and staff need to perform their duties at the college.
Request for Multiple Section Final Exam
Use this form to request a combined final exam time for multiple sections of the same course.
Schedule Template Exemption Form
This form is to be used by chairs when requesting an exemption to the Schedule Template Guidelines.
